The sponsor commitment form is your key to beginning or renewing your sponsorship. The form should be returned via fax 480-289-5765 or via email at info@caepc.org. Your completed and returned commitment form will be date/time stamped. All sponsorships are available on a first response basis. We will honor your preferences as best we can, but please help us by indicating a first and second preference.
